Right to Access

You have the right to inspect and obtain a copy of your health information maintained by Vitalis Intimate Health. To request access, submit a written request to our Privacy Officer at care@vitalishealth.com.

You have the right to request that we amend your health information if you believe it is inaccurate or incomplete. Submit amendment requests in writing to our Privacy Officer.

Right to an Accounting of Disclosures

You have the right to request that we restrict the use and disclosure of your health information. We are not required to agree to your request, except in limited circumstances.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Vitalis Intimate Health or with the Secretary of the Department of Health and Human Services. You will not be retaliated against for filing a complaint.
